Please remember if you child has a food allergy to bring a doctor note yearly stating the allergy in order for it to be flagged at school. If you need to bring medications please bring them to the nurse's office in the original container with the prescription label and sign a medication form.
If your child has Asthma/Anaphylaxis Allergies please come by for a health plan form to be signed by your child's doctor.
